Group Provider Clause Samples
Group Provider is a physician (“Group Physician”), ancillary provider, or other provider with whom Group has entered into a contract for the provision of Capitated Professional Services.
Group Provider. A duly licensed physician, dentist or allied health professional, who practices with Provider as an employee, partner, shareholder, or contractor.
Group Provider. A duly licensed and qualified physician or nurse practitioner who is employed by, or who is a partner or shareholder, of Group.
Group Provider. An individual who is a Provider (as defined in Section 1 herein) and who practices with a Group as an employee, partner, shareholder, member or contractor of the Group.
Group Provider. A duly licensed, certified and qualified health care/ancillary services provider (e.g., nurse practitioner, chiropractor, mental health/substance abuse professional, optometrist, podiatrist, primary care physician, specialist physician, etc…) rendering Provider Services to Members who, when Provider is a Group, is: (a) employed by Provider; (b) a partner or shareholder of Provider; (c) contracted with Provider to provide services under this Agreement.
