GRANT ACCEPTANCE. YOU MUST ACCEPT THE TERMS OF THIS AWARD AGREEMENT WITHIN 60 DAYS OF RECEIPT IN ACCORDANCE WITH THE PROCEDURES SPECIFIED BY THE COMPANY. IF YOU DO NOT ACCEPT THE TERMS AS INSTRUCTED, THIS AGREEMENT WILL AUTOMATICALLY, WITHOUT FURTHER ACTION OF THE COMPANY OR THE ADMINISTRATOR, TERMINATE AND THE AWARD WILL BE FORFEITED AT MIDNIGHT ON THE 60TH DAY. ACCEPTANCE OF THIS AWARD AGREEMENT CONSTITUTES YOUR CONSENT TO ANY ACTION TAKEN UNDER THE PLAN AND THIS AWARD AGREEMENT AND YOUR AGREEMENT TO BE BOUND BY THE COVENANTS AND AGREEMENTS CONTAINED IN ATTACHMENT A AND ATTACHMENT B OF THE PLAN.

GRANT ACCEPTANCE. In consideration of my receipt of the grant of Restricted Share Units as specified on the Grant Report, I acknowledge by accepting the grant (evidenced in writing or through my acknowledgement of the grant through the Company’s third-party equity plan administrator), I agree to the terms, conditions and restrictions set forth in this Agreement and the Plan.
GRANT ACCEPTANCE. The state funding announcement remains an offer until the fully and appropriately executed copy of this Grant Agreement is received by OPB. Upon approval of the Grant Agreement, OPB or its designee will issue a statement of confirmation or acceptance (“funding announcement”) to the Grantee through Grantee’s representative listed in Section 1.20 of this Agreement, upon receipt of which the Grantee may begin submissions to GeorgiaCARES for reimbursement as specified in this Agreement.
GRANT ACCEPTANCE. Subject to the payment of the License Fee in accordance with Article 4, and the due performance by Licensee of its obligations hereunder, and provided that Licensee is not in material breach of its obligations hereunder, Licensor hereby grants to Licensee, a limited, non-exclusive license (except as otherwise specified in the Television License Agreement) to exhibit each Program on a Basic Television Service(s) solely over the Licensed Service(s) in the Territory in the Authorized Language during its License Period, and Licensee shall so license from Licensor such right. Licensee shall exhibit each Program in its entirety. Such exhibition shall be solely on the Licensed Service(s) either directly to Subscribers or to Affiliated Systems and Affiliated Institutions as follows:

Grant recipients are asked to submit a FY2021 YP Grant Acceptance form via the online grantee portal which includes a W-9, vendor survey, and Electronic Funds Transfer form (optional). If you have submitted a W-9, vendor survey, or EFT form within the last 12 months, you are not required to attach them here. Changes to Project Scope Grantees must notify ArtsWave of any substantial changes to the scope of the project as soon as possible. Changes in project scope will be evaluated on a case-by-case basis but may result in a partial or full refund of the grant award.

Progress Report Grant recipients must present a status update to the ArtsWave YP Committee in March 10, 2022 at 12:00 pm. Additional details will be provided before the meeting date. Final Report Grant recipients must submit a Final Report via the grantee portal by July 29, 2022. The final report includes a financial report, an update on the actual results achieved, and lessons learned during program implementation that might be valuable to the broader sector. The final report should also be submitted through the online grantee portal. Failure to submit your final report in a timely fashion may result in ineligibility for future funding from ArtsWave.
GRANT ACCEPTANCE.
