FUNCTIONS & SERVICES. The Services meet the statutory duties of the Partners for the provision of equipment and aids to daily living as per the ▇▇▇ ▇▇▇ ▇▇▇▇ and the Local Authority Social Services ▇▇▇ ▇▇▇▇, and the Health and Social Care ▇▇▇ ▇▇▇▇ and Care ▇▇▇ ▇▇▇▇. This supports the implementation of the Joint Prevention Strategy. The Services to be commissioned under this schedule are:  Hounslow Community Equipment Service  Transforming Community Equipment Service  Small aids and adaptations  Assistive Technology including Telecare The Council agrees to act as Lead Commissioner of the Services in the Scheme Specification. Additional services may be brought within the scope of this agreement during the term by agreement of the Partners.
