FRINGE BENEFITS AND OTHER ALLOWANCES. INSURANCE:
6.1 FULL TIME: The District's monthly insurance contribution for each full-time equivalent (FTE) unit member is one thousand one hundred and forty-five dollars ($1,145.00) per month for the period October 2013 2014 through September 20142015. There will be a one-time transfer of $200,000 from the licensed insurance reserve fund to the District general fund on October 1, 2013, to cover the cost of the insurance increase. There will be a second transfer of $589,000 from the licensed insurance reserves to the District general fund on August 1, 2013, representing the cost of the COLA and a portion of the step increases.
FRINGE BENEFITS AND OTHER ALLOWANCES. 6.1 The District’s monthly insurance contribution for each full time equivalent (FTE) unit member is one thousand one hundred dollars ($1,100) per month for the period October 2010 through September 2011. For 2010-11, the Association will transfer from the licensed insurance reserves $10 per member per month to the District general fund effective October 1, 2010. The District’s monthly insurance contribution for each full time equivalent (FTE) unit member is one thousand one hundred dollars ($1,100) per month for the period October 2011 through September 2012.
FRINGE BENEFITS AND OTHER ALLOWANCES. A. The District shall provide a benefits package that includes full family medical, dental, and vision insurance for each member of the bargaining unit. The District shall contribute up to $1,490.39 per month for such benefits for the 2021-22 benefits year and $1,540.39 for the 2022-2023 benefits year. No residual benefit shall accrue to single employees.
B. Teachers who work less than full time (1.0 FTE) will receive insurance benefits on a pro-rata basis.
C. The current medical insurance carrier is the OEBB. The LCEA shall select the plans available for bargaining unit members per the provisions established by OEBB and this contract. Dental, Vision and Life insurance (long-term disability) will be part of the fringe benefit package offered by LCSD to the bargaining unit. Dental and vision insurance shall be through the District’s self-insured program.
D. Teachers may participate in an IRS Section 125 account by appropriate payroll deductions. The plan to be offered will include accounts for chiropractic care, medical, dental, and vision premiums (above the cap) by way of pre-taxed payroll deductions, and such other accounts as are approved by the District.
E. Members of the Association will participate in the LCSD Insurance Committee to continually evaluate and improve insurance options. Benefit or plan changes require the approval of both the Association and the District’s Board of Directors.
F. For the terms of this Agreement, the parties will designate MODA’s high deductible plan as the preferred district major medical plan for bargaining unit members.
1. Members enrolling in MODA’s high deductible plan for themselves and any eligible family members shall also be eligible to participate in the District sponsored Group Health Reimbursement Arrangement (HRA) plan. Please Note: Due to IRS regulations, Domestic Partners and/or children of Domestic Partners are not eligible for participation in HRA, FSA, or HSA reimbursement.
2. The purpose of the Group HRA is to provide benefits via a claims submission process. 6.1. FULL TIME: The District's monthly insurance contribution for each full time equivalent (FTE) unit member for 2014- 15 is one thousand two hundred and ten dollars ($1,210) one hundred and forty-five dollars ($1,145) per month for the period October 2017 2014 through September 2019, and one thousand two hundred and twenty ($1,220) dollars per month for the period starting October 2019. From October 2017 through September 2020, members will receive subsidies from the licensed insurance reserve fund such that the combined total monthly contribution received by members is one thousand two hundred and forty dollars ($1,240). 2015. For 2015-16, the District’s monthly insurance contribution for each full-time equivalent unit member shall be one thousand, one hundred seventy-five dollars ($1,175) per month. For 2016-17, the District’s monthly insurance contribution for each full-time equivalent unit member shall be one thousand, two hundred dollars ($1,200) month. There will be a one-time transfer of $330,000 from the licensed insurance reserve fund to the District general fund on November 1, 2014, October 1, 2015 and October 1, 2016. During the term of this agreement, the district will not make contributions to the licensed insurance reserve fund on behalf of members who have waived insurance, except that the district will continue to make contributions for Life, LTD, EAP and the Wellness Clinic for such members.

Between October 2017 and September 2020, if members are offered composite rates, members will receive subsidies from the licensed insurance reserve fund such that the combined total monthly contribution received by members is at least one thousand two hundred and fifty dollars ($1,250). If members are offered tiered rates, the JBC will apply a comparable amount of reserve funds to defray the cost of insurance for members electing family coverage. 2015. For 2015-16, the District’s monthly insurance contribution for each full-time equivalent unit member shall be one thousand, one hundred seventy-five dollars ($1,175) per month. For 2016-17, the District’s monthly insurance contribution for each full-time equivalent unit member shall be one thousand, two hundred dollars ($1,200) month. There will be a one-time transfer of $330,000 from the licensed insurance reserve fund to the District general fund on November 1, 2014, October 1, 2015 and October 1, 2016.
