Form of Application. The Contractor shall periodically submit to the Design Professional an Application for Payment on the form set forth in Section 7 (sometimes called a "Periodical Estimate") for each payment requested, and, if requested by the Owner or Design Professional, shall attach backup materials including, but not limited to, receipts or other vouchers, showing his payments for materials and labor, including payments previously made to Subcontractors.

Form of Application. Any individual may apply to take an examination by filing a prescribed form on which he/she states that he/she meets the announced requirements for such examination and that he/she understands that he/she will be eliminated at any stage in such examination if it develops that in fact he/she does not meet the announced requirements. This form must be filed at the office of the Commission on or before the closing date specified in the examination announcement, except that when a sufficient number of applications have not been received up to the closing date, the director may accept applications in the period between the closing date and the scheduled date of the examination. Otherwise applications filed after the closing date may be accepted only on approval of the Commission. When an examination has been announced as a continuous examination as provided in rule 1436, the closing date shall be indefinite and the period of applying shall continue until suspended or terminated by order of the Commission. This rule shall not apply where an examination is held entirely on an oral basis, or where other special circumstances make the procedure not feasible. In such case, each applicant shall file the official application form referred to in rule 1332 before the announced closing date. In promotional examinations, the preliminary forms shall be checked against personnel records, and any applicant who does not meet the announced requirements shall be disqualified in advance.

Form of Application. Design-Builder shall, on or before the twenty-fifth (25th) day of each month, submit to Owner an itemized Application for Payment, completed in accordance with the Contract Documents and the approved Schedule of Values. Each application shall be supported by such substantiating data as Owner may require. When Design-Builder submits its monthly Application for Payment, it shall include, in addition to other requirements of the Contract Documents the following: (a) a completed Affidavit of Amounts Paid Minority and Women’s Business Enterprises (MBE/WBE) form and such other requirements as are identified in Article 22; (b) a completed Statement of Apprentice/Journeyman Participation form and such other requirements as set forth in Article 23; and (c) a waiver and release of claims and mechanic’s liens. Payments will not be considered due and payable by Owner unless these forms are properly completed and timely received by Owner.
Form of Application. Without prejudice to Clause 27.4 (Value of Goods and Materials), each Application for Payment shall specify: the cumulative value claimed by hubco of Works properly executed up to the end of the Relevant Month as set out in Part 11 (Development Amount Analysis) as amended from time to time in accordance with this Agreement; the cumulative amount of any other payments to which hubco claims to be entitled under this Agreement up to the end of the Relevant Month; where Clause 27.6 applies the applicable Retention; and the amount hubco considers is due on the Due Date and the basis upon which that amount has been calculated including a breakdown of the cumulative value of Works properly executed up to the end of the Relevant Month by specific reference to each of the elements set out in the Development Amount Analysis; and the breakdown of the amount hubco considers is due on the Due Date into the separate amounts that hubco considers to be due to: hubco in accordance with this Agreement but excluding the amounts due under Clause 27.3.5 (b), (c) and (d) and, for the avoidance of doubt, the Project Development Fee; the Contractor in accordance with the Construction Contract but excluding the amounts due under Clause 27.3.5 (c) and (d); each PBA Sub-Contractor in accordance with its Sub-Contract; and each Non-PBA Sub-Contractor in accordance with its Sub-Contract.
