Fee-based Services. The Commission will ensure that any fee-based services provided by the Commission are set forth in a services agreement that is separate from the charter agreement and ensure that the purchase of such services is not and will never be a condition of charter approval, continuation, modification or renewal.

Fee-based Services. The following services (the “Fee-Based Services”) are offered by the Clinic, but involve additional expenses not included within the Membership Fees and incur additional charges. All charges for Fee-Based Services are due and payable at the time the service is provided. a. EKGs b. Cryotherapy (freezing) for wart treatment c. Cryotherapy (freezing) of skin lesions d. Shave skin biopsies e. Skin tag removal f. Simple laceration repairs (i.e. stitching or skin glue for minor cuts) g. Suture removal h. IUD removal i. Incision and drainage of minor abscesses j. Nebulizer treatments

Fee-based Services. To establish a relationship between the County and the Agency whereby the Agency provides the County the following “Fee-Based Services”: Training for Parents/Staff; Technical Assistance for Head Start Staff; and Individual Service Plan Meetings.

Fee-based Services. The Staffing Plan shall include the following, if the method of payment for the performance of services, or any portion thereof, is through Fee(s): Required titles of Key Personnel and specific individual for each title, as identified by the Consultant in its Proposal for the Contract and set forth in Exhibit B.

Fee-based Services. If you subscribe to any fee-based FieldView Services, your terms of service include these general Terms of Service, plus any Additional Terms relating specifically to such fee-based services disclosed to you at the time of subscription, including but not limited to the prices, charges, and terms and conditions provided to you in marketing and informational material associated with the FieldView Services or on the Climate website, all of which are hereby incorporated by reference. Your order for fee-based FieldView Services will become effective only if we accept your order. You agree to pay when due, all amounts set forth in the applicable order for any fee-based FieldView Services and, at the time of renewal, the applicable subscription fees for any renewed FieldView Service subscription, plus applicable taxes (unless you provide us with a valid tax exemption certificate authorized by the appropriate taxing authority). If your subscription fee payment is overdue, we may suspend your account access until such amounts are paid in full. Unless otherwise specified, overdue amounts will accrue interest at the lesser of 1.5% per month or the highest rate allowed by law.

Fee-based Services. 1. It shall be possible to use the Service (except Fee-based Services) for free, but the Customer may decide to use additional Fee-based Services. 2. Fee-based Services can only be used as long as the Service(s) are provided. After the termination of the Service(s), the Customer cannot continue to use any Fee-based Services. 3. The pricing of the Fee-based Services shall be described before any purchase can be made. The price for the desired Fee-based Services will be shown in Euros, US dollars, or in another currency applicable to the Customer’s region. Cygames or a settlement agency (including but not limited to an intermediary platform holder) will have the right to amend or change the prices prospectively. If a price determined by Cygames or a settlement agency is amended, the amended price shall be displayed. 4. Customers can purchase Fee-based Services by selecting the desired Fee-based Services, selecting the desired payment method, and press the relevant button to confirm their intention to purchase the Fee-based Service, and which concludes the purchasing process. 5. The payment options may vary according to the Customer’s region and the technical feasibility of the payment options available in the market. 6. The money will be collected via the respective platform as specified at the point of purchase. 7. Statutory interest shall accrue in instances of default. 8. The Customer shall not be able to transfer items obtained in a Fee-based Services to another Account or sell or transfer their Account. 9. If a minor uses such Fee-Based Service(s) under the Account of an adult, valid power of representation and the consent of such adult shall be required in order for the minor to use the Service(s) (including their consent to this Agreement, to the paragraphs in this article, and to the Individual Service Agreement) and only they shall be the contractual party of Cygames, bound by this Agreement. 10. If a minor of the age of 13 or above uses the Service, the consent of a legal representative such as a parent is necessary for all use of the Service, including use of Fee-based Services such as purchase of items and services (including consent to this Agreement and Individual Service Agreements). 11. If a Customer who was a minor at the time of entering this Agreement or an Individual Service Agreement uses the Service after reaching adulthood, the Customer is deemed to have consented to all agreements relating to the use of the Service through the continu...

Fee-based Services a) On-Site Nurse Navigator Parents and Parents-in-Law will have access to an Onsite Nurse Navigator to attend physician appointments relating to their cancer diagnosis. This personalized one-to-one service helps to ensure nothing is left to chance and all questions and concerns are addressed. A cancer diagnosis is required for the Onsite Nurse Navigator. Parents and Parents-in-Law will have access to an Onsite Nurse Navigator appointment for an additional per appointment fee. b) Cancer DNA Testing Parents and Parents - in - Law of Participants and Dependent will have access to comprehensive DNA testing for cancer if they become diagnosed with cancer while enrolled in the Cancer Guardian program. Testing is subject to treating physician approval based on clinical need and necessity. Report results are delivered back to the treating physician via secured physician portal in 10-14 days from the date that the test specimen is received at the lab. Complex cases may require additional time for report delivery. Section C - Cancer Incidence Procedures

Fee-based Services. As an alternative, and for an additional fee, service may be available Outside of Principal Period of Maintenance subject to the availability of personnel, among other factors. The service alternative you have chosen is recorded on your invoice. This service does not cover any related network problems or any Service which would be unique to the System’s operation on a network. A service technician will be dispatched to your location to service your System, according to your service level, as follows: Holidays: Regular holidays shall include New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day, unless you are notified otherwise by QUALXSERV. An Adult Must Be Present At Residences: For On-Site Service provided at a residence, an adult must be present at all times during the service technician’s visit.
