Applicant disclosed that her license to practice psychology in Minnesota is currently suspended due to a Stipulation and Consent Order adopted by the Minnesota Board of Psychology on June 26, 2020. 5. The suspension of Applicant’s Minnesota psychologist license stemmed from a pattern of failure to complete evaluation reports in a timely manner and providing the Minnesota Board with false information indicating that her evaluation reports were up-to-date.
