Common use of Documents Required from Subscriber Clause in Contracts

Documents Required from Subscriber. 3.1 The Subscriber must complete, sign and return to the Company: (a) an executed copy of this Subscription Agreement; (b) a Prospective Investor Suitability Questionnaire in the form attached as Exhibit B (the "US Questionnaire"); and (c) a British Columbia Accredited Investor Questionnaire in the form attached as Exhibit C (together with the US Questionnaire, the "Questionnaires"). 3.2 The Subscriber shall complete, sign and return to the Company as soon as possible, on request by the Company, any documents, questionnaires, notices and undertakings as may be required by regulatory authorities and applicable law.

Documents Required from Subscriber. 3.1 The Subscriber must complete, sign and return to the Company: (a) an executed copy of this Subscription Agreement; (b) a Prospective Investor Suitability Questionnaire in the form attached as Exhibit B A (the "“US Questionnaire"”); and (c) a British Columbia Accredited Investor Questionnaire in the form attached as Exhibit C B (together with the US Questionnaire, the "“Questionnaires"”). 3.2 The Subscriber shall complete, sign and return to the Company as soon as possible, on request by the Company, any documents, questionnaires, notices and undertakings as may be required by regulatory authorities authorities, the OTC Bulletin Board and applicable law.

Documents Required from Subscriber. 3.1 The Subscriber must complete, sign and return to the Company: (a) an executed copy of this Subscription Agreement; (b) a Prospective Investor Suitability Questionnaire in the form attached as Exhibit B C (the "“US Questionnaire"”); and (c) a British Columbia Accredited Investor Questionnaire in the form attached as Exhibit C D (together with the US Questionnaire, the "“Questionnaires"”). 3.2 The Subscriber shall complete, sign and return to the Company as soon as possible, on request by the Company, any documents, questionnaires, notices and undertakings as may be required by regulatory authorities authorities, the OTC Bulletin Board and applicable law.

Documents Required from Subscriber. 3.1 The Subscriber must complete, sign and return to the CompanyCompany the following documents: (a) an two (2) executed copy copies of this Subscription Agreement; (b) a Prospective Investor Suitability Questionnaire in the form attached as Exhibit B (the "US Questionnaire"); and (c) a British Columbia an Accredited Investor Questionnaire in the form attached as Exhibit C (the "U.S. Questionnaire"); and (c) an Investor Questionnaire (together with the US U.S. Questionnaire, the "QuestionnairesQuestionnaire").) in the form attached as Exhibit D. 3.2 The Subscriber shall complete, sign and return to the Company as soon as possible, on request by the Company, any additional documents, questionnaires, notices and undertakings as may be required by any regulatory authorities and applicable law.

Documents Required from Subscriber. 3.1 The Subscriber must complete, sign and return to the Company: (a) an executed copy of this Subscription Agreement;; and (b) for U.S. residents, a Prospective Investor Suitability Questionnaire prospective investor suitability questionnaire in the form attached as Exhibit B A (the "US “U.S. Securities Law Questionnaire"); and (c) a British Columbia Accredited Investor Questionnaire in the form attached as Exhibit C (together with the US Questionnaire, the "Questionnaires"”). 3.2 The Subscriber shall complete, sign and return to the Company as soon as possible, the Subscription Agreement, and the U.S. Securities Law Questionnaire, and on request by the Company, any documents, questionnaires, notices and undertakings as may be required by regulatory authorities and applicable law.

Documents Required from Subscriber. 3.1 The Subscriber must shall complete, sign and return to the Company, as soon as possible: (a) an executed copy of this Subscription Agreement;, (b) a Prospective fully completed and executed Accredited Investor Suitability Questionnaire (the “Questionnaire”) in the form attached hereto as Exhibit B (the "US Questionnaire"); andB; (c) a British Columbia Accredited Investor fully completed and executed Beneficial Ownership Questionnaire in the form Form attached hereto as Exhibit C C; and (together with the US Questionnaire, the "Questionnaires"). 3.2 The Subscriber shall complete, sign and return to the Company as soon as possible, d) on request by the Company, any other documents, questionnaires, notices and undertakings as may be required by the Company in order to enable or show compliance with the requirements of regulatory authorities and applicable law.
