Common use of Designated Record Set Clause in Contracts

Designated Record Set. To the extent that Business Associate maintains PHI in a Designated Record Set and at the request of Covered Entity, Business Associate agrees to provide access to PHI in a Designated Record Set to Covered Entity or, as directed by Covered Entity, to an Individual in order to meet the requirements under 45 C.F.R. § 164.

Designated Record Set. To the extent that Business Associate maintains PHI in a Designated Record Set and at the request of Covered EntitySet, Business Associate agrees to provide access to PHI in a Designated Record Set make available to Covered Entity orsuch information as Covered Entity may require to fulfill its obligations to provider access to and amendment of PHI pursuant to the Privacy Rule, as directed by Covered Entityincluding, to an Individual in order to meet the requirements under but not limited to, 45 C.F.R. CFR § 164164.524 and 164.526.

To the extent that Business Associate maintains PHI in a Designated Record Set and at the request of Set, make available to Covered Entity, Business Associate agrees Entity such information as Covered Entity may require to fulfill its obligations to provide access to and amendment of PHI in a Designated Record Set pursuant to Covered Entity orthe Privacy Rule, as directed by Covered Entityincluding, to an Individual in order to meet the requirements under but not limited to, 45 C.F.R. § 164CFR §164.524 and 164.526.

To If the extent that Business Associate maintains has PHI in a Designated Record Set and at Set, the request of Covered Entity, Business Associate agrees to provide access of those records to PHI in a Designated Record Set to Covered Entity or, as directed by the Covered Entity, whenever requested by the Covered Entity. If the Covered Entity or the Individual so request, the Business Associate shall make requested amendments to an Individual PHI contained in order to meet the requirements under 45 C.F.R. § 164Designated Record Set.
