Deaths. 1) In the event of a death of a Contract Agency inmate in the County jail, the Contract Agency shall be promptly notified by telephone or fax with a follow-up notification in writing via US mail. Xxxxx County Sheriff’s Office and the Xxxxx County Coroner will investigate the circumstances. The Contract Agency may, if it wishes, join in the investigation and receive copies of records and documents in connection with the investigation.
Deaths. If reasonable notice is given to his/her supervisor, an employee shall suffer no loss of regular pay for a reasonable amount of scheduled time lost on account of death in the immediate family or household of such employee.

In no case will payment be granted in lieu of time off nor will payment be made if death and funeral occur during non-work time. If a death or funeral of a member of the employee’s immediate family occurs during the employee’s vacation, the employee will be allowed to use funeral leave and reschedule the remainder of his/her vacation if time and service requirements permit.

All AEs resulting in death either during the study period or within 12 weeks (84 days) after the last dose of IMP, must be reported as an SAE within 24 hours of the investigator’s knowledge of the event. The convention for recording death is as follows: • AE term: lead cause of death (e.g. multiple organ failure, pneumonia, myocardial infarction) • Outcome: fatal. The only exception is if the cause of death is unknown (i.e. sudden or unexplained death), in which case the AE term may be “death” or “sudden death”.

The Contract Agency may, if it wishes, join in the investigation and receive copies of all records and documents in connection with the investigation. Lewis County will promptly comply with all state and local requirements regarding notifications and investigations.

In the event of a death of an employee’s wife, husband, son, daughter, mother, father, brother, sister, grandmother, grandfather, grandfather-in-law, grandmother-in-law , mother-in-law, father-in-law, stepmother, stepfather, stepchild(xxx), son-in- 35 xxx, daughter-in-law, brother-in-law, sister-in-law, granddaughter, or grandson, any such employee scheduled to work shall be excused for a period not to exceed three (3) days beginning with the date of the death and ending with the day after the funeral, without loss of pay at straight time rates for the hours such employee was scheduled to work. Father and mother shall include persons who have served as parents. When the relative is a spouse, parent or child of the employee, two (2) days with pay will be automatically given if requested. In the case of death of any person living in the household of the employee and not specifically referred to above, absence with pay shall be allowed for the necessary time on the day of the funeral to attend the funeral. Additional leave without pay will be granted where necessary and requested. When an employee serves as pallbearer for a fellow employee, necessary time off not to exceed one (1) day, shall be granted without loss of pay at straight time rates for the hours such pallbearer was scheduled to work.
