Data Reporting/Deliverables. AGENCY shall submit to THE PARTNERSHIP, in accordance with schedule outlined in the Scope of Work (Exhibit A).
Data Reporting/Deliverables. On a requested basis SEDNET shall submit to THE PARTNERSHIP all data, performance outcomes and reports on work performed to include, but not limited to: SEDNET will notify and/or refer all Family Services Planning Team (FSPT) children/youth within the SEDNET Network who are exiting crisis stabilization units to the “System of Care” thru the completion of: System of Care Referral Form Information Release CFOI/NOMS SEDNET will complete the SAMHSA required UF-CHEQR Training on CFOI/NOMS. SEDNET will collaborate with the “System of Care” on an ongoing basis. SEDNET will uphold and implement the “System of Care” core values of family-driven, youth-guided and cultural and linguistic competency. SEDNET will engage in data sharing as requested for SAMHSA TRAC reporting system in relation to the relevant TRAC goals.
