Cremation Services. (a) This agreement gives you the right to receive the cremation services from us as set out in these terms and conditions. (b) We will take all reasonable steps to make sure that our cremation services meet any religious or cultural requirements outlined in the contract.
Cremation Services. The cremation services set out in this Annexure C form part of the agreement between Mountain View Crematoria Pty Ltd (we, us) and the applicant (you, your).

Cremation Services. Unless otherwise directed by the Division or Medical Examiner’s Office, or unless the body is unidentified, all unclaimed bodies shall be cremated, except as noted in Section 21, Services for Veterans. For cremated eligible decedents, the Contractor shall provide the Division’s representative a completed original “certification of cremation”. The certification of cremation shall include the name and address of the crematory, the name of the cremated decedent, the name of the funeral home or cremation company handling the body, the cremation date, and the name(s) of the person(s) who conducted the cremation. All information and documentation (including cremains, if requested by the Division) shall be submitted by the Contractor to the Division’s representative at the conclusion of cremation services. Cremations shall be performed according to the laws, rules, and regulations of Orange County and of the State of Florida. When a next of kin has not been located, a written authorization for cremation services shall not be issued for a minimum of thirty (30) days from the date of application. At the option of the Division, the 30-day period may be extended to allow for additional investigation by the authorities or other officials, or the period may be shortened to less than 30 days, when the circumstances allow. A casket, embalming, and clothing are not required nor covered for a cremation service, except that embalming may be authorized by the Division for cases expected to need storage of 30 days or more. If the Division anticipates requiring 30 days or more to investigate or seek family, the Division will advise the Contractor to embalm. A Notice to Proceed will be issued within five (5) working days, or as soon as possible thereafter. Approved cremation services shall include, but are not limited to, pick up of the decedent’s remains from the Medical Examiner’s Office or other appropriate facility, preparation of the remains in accordance with industry standards for cremation, and transportation of the remains to the crematory, where the decedent’s remains shall be cremated. At the option of the Division, a representative of the Division may attend any authorized cremation by giving notice to the Contractor and requesting that the cremation service be conducted at a time that would allow attendance during the normal work hours of the Division’s representative.
