Coverage Limited to Program Coverage. Except as otherwise provided under this Contract, all health care services prescribed or recommended by a participating physician, dentist, care manager, or other practitioner, or approved by the HEALTH PLAN are limited to services that are covered under MA, GAMC, or MinnesotaCare.
