CONTRACTOR'S REFERENCES. (Not in excess of two pages) Provide references for at least three of the Contractor’s clients for which the Contractor has provided services, similar to those described in this RFP. Reference information should include:
(i) the name of the client; (ii) the name, address and telephone number of the client’s contact person for purposes of the Contractor’s services to the client; (iii) a description of the type and scope of services provided to the client; (iv) the date(s) the Contractor provided the services to the client.
CONTRACTOR'S REFERENCES. Contractor shall demonstrate a minimum of (5) years’ experience by providing references acceptable to the City. Please provide a list of five (5) agencies or companies for whom you have successfully provided work on projects of similar scope. Providing vendor’s reference sheet in lieu of this form will disqualify vendor.
