Contract Providers. To the Knowledge of the Borrower, no Contract Provider is: (a) a party to any judgment, order, decree, agreement or instrument, or subject to restrictions, which could individually or in the aggregate reasonably be expected to have a Material Adverse Effect; (b) in default in the performance, observance or fulfillment of any of the obligations, covenants or conditions contained in any Medicaid Provider Agreement, Medicare Provider Agreement or other agreement or instrument to which such Person is a party, which default has resulted in, or if not remedied within any applicable grace period could result in, the revocation, termination, cancellation or suspension of the Medicaid Certification or the Medicare Certification of such Person, which revocation, termination, cancellation or suspension could reasonably be likely to have a Material Adverse Effect; or (c) has been convicted of an offense or has committed an act or omission which could reasonably form a basis under 42 U.S.C. § 1320a-7 and any statutes succeeding thereto and regulations promulgated thereunder for the Secretary of HHS to exclude the Contract Provider from participation in a Federal health care program, which exclusion from participation could reasonably be likely to have a Material Adverse Effect.

Contract Providers. Fallon Health ▇▇▇▇▇▇▇▇-▇▇▇▇ contracts with community providers when necessary. Included as an insert in your enrollment packet is a list of contracted community providers. This list is updated monthly as providers are added or removed. You may request a copy of the contracted community provider list at any time. All services provided by contracted providers must be pre-approved by your interdisciplinary team.
Contract Providers. HCCC establishes and maintains various AFFORDABLE Medical Networks through Provider Agreements with Contract Providers for the delivery of Medical Services at negotiated rates. HCCC may at any time and in its sole discretion add or delete any Contract Provider(s) from any of its AFFORDABLE Medical Networks.

Contract Providers. Contract Providers means Participant’s Eligible Professionals, and/or employees or contractors of Participant, (i) who have agreed to provide services to beneficiaries and who ▇▇▇▇ for items and services furnished to Medicare fee-for-service beneficiaries under the Medicare billing number assigned to the TIN of Participant, and (ii) who are participating in this ACO via the Participant’s execution of this Agreement.
