Common use of Community Based Care Programs Clause in Contracts

Community Based Care Programs. If the community in which the Health Plan operates has a community-based care program contracted by DCF for the provision of children’s protective services, the Health Plan shall determine how to provide services to Enrollees served by the community-based care program. The Health Plan shall develop, during the implementation phase of the Contract, or upon notification that the department has contracted with a Health Plan, a cooperative agreement between the Health Plan and the community-based care program. Medicaid and DCF shall approve the agreement. The Health Plan shall be prepared to provide services in a collaborative manner in each county covered by the Health Plan.
