CHOICES Program. If Provider renders Covered Services to Covered Persons under TennCare’s program for Long-Term Services and Supports for elders and/or persons with physical disabilities, Provider shall facilitate notification of the Covered Person’s Care Coordinator by notifying Subcontractor in accordance with Subcontractor’s and Health Plan’s processes, as expeditiously as warranted by the Covered Person’s circumstances, of any known significant changes in the Covered Person’s condition or care, hospitalizations, or recommendations for additional services.
CHOICES Program. If Provider renders Covered Services to Covered Persons under TennCare’s program for Long-Term Services and Supports for individuals 65 and older and/or persons with physical, intellectual, or developmental disabilities, Provider shall notify United, in accordance with United’s processes, as expeditiously as warranted by the Covered Person’s circumstances, of any known significant changes in the Covered Person’s condition or care, hospitalizations, or recommendations for additional services. United shall in turn notify the Covered Person’s Care/Support Coordinator/ISC/DIDD Case Manager.
