The "Case Plans" clause outlines the requirement for creating and maintaining structured plans for managing individual cases within a legal or service context. 4.3.1 Assess FAMILY needs and collaborate with ADMINISTRATOR to develop individual case plans inclusive of clear, measurable goals and outcomes, directly connected to CDSS and HomVEE guidelines. Assessment of FAMILY needs should be ongoing throughout the duration of participation in the program. The case plan should build on the strengths established during the home visiting period, and the FAMILY’S associated connections to child care. Case plans will also identify Material Goods needed by the participating families up to $500 per FAMILY for the duration of their participation in HVP.
4.3.2 Monitor progress in achieving case plan objectives, and ensure the provision of all services specified in the case plan.
