Board Notification/Approval. 11 Prior to the administration of a vaccine to an individual under this Agreement, Authorizing Practitioner must submit a copy and receive approval of this Agreement from the Alaska State Medical Board prior to the administration of a vaccine under this Agreement. Authorizing Practitioner must also submit a Cooperative Practice Agreement Application.
