Authorization Forms. Driver authorizations forms for both county and private cars shall be developed and revised as necessary by the Human Resources/Risk Management Department. All Department Heads or their designated representatives shall use the form(s) designated by Risk Management.
Authorization Forms. The United Credit Union shall submit authorization cards for deduction upon forms approved by CPS.
Authorization Forms. The Union will supply the Company with Initiation Deduction Authorization Forms, Application for Membership Forms, Dues Deduction Authorization Forms and Health and Welfare Enrolment Forms, all of which shall be signed by all new employees on the date of hire. The Company will forward all completed Application for Membership Forms and Health and Welfare Forms to the Union. All forms shall be returned to the Union within thirty (30) calendar days from the date of hire.
Authorization Forms. The City may request verification of employee’s authorization form giving affirmed consent to deduct dues or fees only if a legitimate dispute arises about the existence or term of the authorization.
Authorization Forms. Authorization for dues deduction shall be submitted to the Board Office on the form set forth in annexed Schedule B.
Authorization Forms. The Employer shall present new employees with a payroll deduction authorization and membership form(s), which shall be supplied by the Union. Employees electing to contribute additional contributions to the Committee on Political Education (COPE) shall require a separate authorization for this deduction. The Employer shall be the agent for receiving such monies and the deduction of such monies by the Employer shall constitute payment of such monies by the employee. In cases where the Employer has not submitted a payroll dues authorization form from an employee and did not report the matter to the Union within thirty-one (31) calendar days of the date of hire, the Employer will be liable for the amount of the deduction.
Authorization Forms. In the event an employee has not submitted the authorization for deduction of Union dues as provided in Section 5.1 (Deductions) above but has not withdrawn an authorization for deduction of Union dues previously on file with the auditor of the City, the old form of authorization shall remain in effect until it has been withdrawn or replaced by the form specified in Section 5.1 (Deductions); provided, however, that the auditor will accept new authorizations for deduction of Union dues only as provide in Section 5.1 (Deductions).
Authorization Forms. The Department of Financial Services shall furnish a uniform travel authorization request form which shall be used by all state officers, employees, and authorized persons when requesting approval for the performance of travel to a convention or conference. The form shall include, but not be limited to, provision for the name of each traveler, purpose of travel, period of travel, estimated cost to the state, and a statement of benefits accruing to the state by virtue of such travel. A copy of the program or agenda of the convention or conference, itemizing registration fees and any meals or lodging included in the registration fee, shall be attached to, and filed with, the copy of the travel authorization request form on file with the agency. The form shall be signed by the traveler and by the traveler's supervisor stating that the travel is to be incurred in connection with official business of the state. The head of the agency or his or her designated representative shall not authorize or approve such request in the absence of the appropriate signatures. A copy of the travel authorization form shall be attached to, and become a part of, the support of the agency's copy of the travel voucher.
