ASSURANCE PROGRAM. The Contractor shall provide and maintain a quality assurance program acceptable to the Government, and shall perform or have performed the inspections and tests pursuant to that program to substantiate that the material and workmanship provided pursuant to the job order conform to the drawings, specifications, job orders, and contract requirements listed herein. The Contractor's quality assurance program shall be in accordance with that set forth in NSI 009-04. In addition to its rights under clause 252.217-7005 “Inspection and Manner of Doing Work”, the Government shall have the right to deem work or material furnished by the Contractor to be incomplete and not in accordance with the requirements of the job order by reason of the Contractor's failure to comply with the requirements of its quality assurance program and, as such, the Contractor shall not be entitled to progress payments for said work and material (See CDRL A006).

ASSURANCE PROGRAM. It is agreed that Quality Assurance is the domain of UPFRO who at their own discretion, will implement ongoing Quality Assurance Programs including but not limited to; management review, telephone checks, and in-field re-checks in order to assure the quality of reports. FSV and UPFRO may agree upon any additional Quality Assurance Programs and reasonable rates within the course of this relationship.

The City routinely reviews recordings and incident histories that are requested by: law enforcement agencies, attorneys and the public as part of a Public Records Act request and provides feedback or takes corrective action, as appropriate. In addition, the City has occasionally performed spot checks of incident histories and recordings. The parties have implemented a systematic Quality Assurance (QA) Program that provides for regular random checks with feedback to improve the performance of the Dispatch Center and its employees.

Terracon believes that all project documents rendering professional opinions or recommendations provided to clients should not rely solely on the judgment of one individual practitioner. A secondary review of a practitioner’s work improves the overall quality of the opinion or recommendation. Therefore, Terracon project documents rendering professional opinions, conclusions, or recommendations must be reviewed and signed by an Authorized Project Reviewer prior to submittal to the client.

ASSURANCE PROGRAM. The contractor must formally adopt a quality assurance program for enrollees. The quality assurance program must include written goals, policies, and procedures that ensure enhancement of quality of life for enrollees, emphasize quality patient outcomes, and, to the extent feasible, promote the coordination of acute and long-term care services. The quality assurance program must have a system to identify and prioritize problem areas for resolution and a process to design and implement strategies to resolve identified problems. The system must include: a process for changing the current quality assurance program as needed; a protocol that dictates the active involvement of the medical director, the quality assurance director, medical/clinical providers, and the director of the program; and a description of the mechanism for measuring the success of quality assurance strategies and for providing feedback to all providers involved in the program. Specifically, the contractor must have a quality assurance program which includes the following: A. A written description of the quality assurance program. B. Written responsibilities of the governing body for monitoring, evaluating, and improving care. C. A procedure for quality assurance program supervision. D. Assurance of adequate resources to carry out the program's specified activities effectively. E. A protocol for provider participation in the quality assurance program. F. A procedure for delegation of quality assurance responsibilities to designated personnel. G. A procedure for credentialing and re-credentialing providers. H. A procedure for informing enrollees about their rights and responsibilities. I. Assurance of availability of and accessibility to services and care. J. A procedure to ensure the accessibility and availability of medical and long-term care records, as well as proper record keeping, and a process for record review. K. A procedure for utilization review. L. A procedure for quality assurance program documentation. M. A procedure for coordination of quality assurance activities with other management activities. N. A continuity of care system. O. An active quality assurance committee.
ASSURANCE PROGRAM. All Suppliers to GVC must hold a current Dairy Licence which requires them, among other things, to implement and maintain an Approved Quality Assurance Program. Suppliers may choose to comply with an alternative Quality Assurance Program which has been approved by relevant government regulator of the state in which the farm is located. Any Supplier that fails an audit (Critical Non-conformance) could be immediately suspended from supply until the cause of the failure is rectified. Suppliers who have a minor or major non-conformance entered on their audit will be given time to rectify the issue. GVC is available for assistancein implementing an on-farm quality system. For purposes of compliance, GVC has developed QA program which GVC Suppliers are required to implement. All forms and manuals are available from GVC.
