Annual Notification. Unit members on Category I long-term leaves must notify the Human Resource Services Division on or before March 1 of each year of the leave as to their intention either to extend the leave for another school year (if eligible), to return to paid service at the beginning of the next school year, or to resign from the District.
Annual Notification. The Board agrees that unit members will be notified as to the number of accumulated sick leave days annually via their employee portal.

Annual Notification. We may communicate with you before the end of your Price Plan to let you know that your Commitment Period is coming to an end and the options available to you. Once the Commitment Period of your Price Plan expires, you will be placed on our standard Variable Price Plan or an alternative Price Plan, at our discretion. Before you are placed on an alternative Price Plan (excluding our standard Variable Price Plan), we will communicate with you to let you know the details of the alternative Price Plan (which shall include a fixed Commitment Period) that we intend to place you on following the expiry of your existing Commitment Period. You will have 3 weeks to contact us to object to being placed on the alternative Price Plan that we have proposed. If we do not hear from you within this period, you will automatically be placed on the new Price Plan following the expiry of your Commitment Period. In such circumstance, these Conditions shall apply.
Annual Notification. The Superintendent will, within ten (10) school days of the beginning of each school year, notify all nonparticipating eligible staff of the Sick Bank, and the opportunity to participate. Eligible staff must sign up for participation within ten (10) school days after notification by the Superintendent.

Once a year, Contractor must notify members in writing of their rights to request and obtain the information listed below: • Names, locations, telephone numbers of, and non-English languages spoken by current contracted providers in the member’s services area, including those not accepting new patients. • Any restriction on the member’s freedom of choice of network providers • Member rights and protections, including those specified in 42 C.F.R. § 438.100 • Notify all members of their disenrollment rights • Information on grievance, appeal, and State Fair Hearing procedures, including applicable time frames and the information specified in 42 C.F.R. § 438.10(g)(1) • The amount, duration, and scope of benefits available under this Agreement in sufficient detail to ensure that members understand the benefits to which they are entitled • Procedures for obtaining benefits, including authorization requirements • The extent to which, and how, members may obtain benefits from out- of-network providers • The extent to which, and how, after-hours and emergency coverage are provided, including: The fact that prior authorization is not required for emergency services. The process and procedures for obtaining emergency services, including use of the 911-telephone system or its local equivalent. The member has a right to use any hospital or other setting for emergency care. • Cost-sharing, if applicable • Additional information that is available on request, including information on the structure and operation of the Dental Plan and provider incentive plans as set forth in 42 C.F.R. § 438.6(h). Contractor agrees to submit to EOHHS for prior review and approval the written materials to be used to fulfill these requirements in accordance with Guidelines for Marketing and Member Communication Materials for Rhode Island’s Medicaid Managed Care Programs.
