Common use of Annual Affidavit Clause in Contracts

Annual Affidavit. 1. The adoptive parent(s) shall submit the Virginia Annual Affidavit for Adoption Assistance to the local department annually, within thirty (30) days of the anniversary date of the effective date for this executed agreement. The annual affidavit shall be signed by at least one (1) of the adoptive parent(s). 2. The local department shall notify the adoptive parent(s) in writing two months before the annual affidavit is due.
