Common use of Allocation of Enrollees Report Clause in Contracts

Allocation of Enrollees Report. The provider shall submit a quarterly report that details the total number of recipients, by eligibility category and separated by age group, assigned to each mental health care provider in accordance with section 10.11.1, and Table I, Allocation of Enrollees, attached hereto and made a part of the contract. This report shall include the following data: • Total number of recipients, by age group, in each Medicaid eligibility category assigned to each provider (age groups are children under age 21, and adults age 21 and older); • Total number of recipients, by age group, assigned to each provider; • Percentage of total recipients in the plan assigned to each provider; • Total number of recipients by age group enrolled in the plan.
