The "Agency or Program Modifications" clause allows for changes to be made to the scope, structure, or operation of an agency or program after an agreement has been established. Agency or Program Modifications. The AGENCY agrees to report in writing any changes related to this program in administrative staffing and/or changes on the Board of Directors, AGENCY composition (including, but not limited to, AGENCY name change, resignation, and/or termination of AGENCY’s Executive Director, President/CEO, and/or merger acquisition). The AGENCY shall provide written notification of any such changes to the COUNTY’s Manager of the CCC or designee within five
