The employer, the medical facility, and the testing laboratory agree that the results of the described tests are to be held in strictest CONFIDENCE between the employer and the medical facility. It is further agreed that the results of the above tests will not be provided to anyone other than the designated employer contact person without the express written consent of the employee.
