Admission Data. Prior to providing services under this Contract, Client shall provide all information required by Facility, including, but not limited to: complete name, address, telephone number, Social Security number, Medicare number, and date of birth of Client; complete name, business address, home address, telephone number of person to contact in the event of an emergency; name of family physician; name of physician to call in an emergency.
