Sick Leave Documentation. All employees may be required to submit medical documentation after three (3) consecutive work days of illness. All employees will be required to submit medical documentation after five (5) consecutive work days of illness.

Reasonable medical evidence of incapacity may be required for sick leave use of forty-eight (48) hours or less duration, and shall be required for sick leave use for more than forty-eight (48) hours duration.

Sick leave shall be recorded regularly in the personnel records. The Chief of Police or his designee shall review all sick leave records periodically and shall investigate any causes, which indicate abusive privilege.

An incidence of absence is defined as one (1) day of nonattendance which invokes the use of a sick day or results in an unpaid absence if no sick days are available, except for an approved Family Medical Leave. In cases of absences of three (3) or more consecutive work days due to illness or injury, the District may require a physician’s statement certifying that the teacher’s condition prevented him/her from appearing for work and indicating that the teacher is fit to return to full duty. If the District requires a physician’s statement from a teacher, the District will reimburse the teacher for the teacher’s examination by the teacher’s physician if the physician determines that the teacher’s condition prevented him/her from reporting for work, to the extent that the teacher’s examination is not covered by insurance. In such a circumstance, the District’s obligation to reimburse the teacher will be limited to the cost of examination as well as any accompanying tests or procedures as part of the examination.

Falsification of either a written, signed statement or a physician's certificate shall be grounds for disciplinary action including dismissal.

The medical certificate must describe the nature, and if known, the anticipated duration of the illness.
