Case Report Form definition

Case Report Form means a printed, optical or electronic document or database designed to record all of the information, which is required by the Protocol to be reported to the Sponsor on each Study Participant.
Case Report Form or “CRF” means and includes any document - printed, optical, electronic or others - designed to record all Protocol information required to be reported to the Sponsor for each patient participating in the Study. The CRF shall, as much as possible, be reported by electronic way between the Parties. The Parties agree that CRF’s data reported electronically shall have the same probative force as scriptural data or signature;
Case Report Form means the form (whether paper or electronic) for collecting certain data about each Subject, including the data collected for such Subject.

  • For each pivotal trial, provide a sample annotated Case Report Form (or identify the location and/or provide a link if provided elsewhere in the submission).

Case Report Form or "CRF" means the record of pertinent information collected on each subject who participates in the Study;
