Enclosed is the Dental Plan Group Agreement effective January 1, 2015 through December 31, 2015 for Marion County MCLEA. There are two documents which serve as the entire contract. The Wrap (titled “Group Agreement”) contains the group contract provisions, including rates. The Evidence of Coverage (EOC) is the member portion of the contract. The EOC contains benefit descriptions, limitations, exclusions, and instructions which assist the member in obtaining care.
