Utilization Report. The CONTRACTOR shall submit a monthly utilization report that provides a summary of information on NEMT services provided to members. The report shall include, at minimum, by mode of transportation: the number of trips, number of unduplicated members, and number of miles.
Utilization Report. CONTRACTOR shall provide a monthly report of passenger 6 utilization of the Transportation Services provided under this Agreement, due no later than twenty (20) 7 calendar days following the end of the month being reported. Utilization Report. The Customer is provided with an on-line access for monitoring the utilization of its connections which are connected to the International Service Network. The utilization reports may be viewed via the Web. The reports will be daily, weekly, monthly and annual. The Customer will be provided with a username and password for the on-line access.
Utilization Report. Within five (5) business days after each calendar month-end, ARGO shall provide LEVOIP with a Utilization Report as described in the Section 6.1.1, Reporting, detailing the utilization of LXXX.XX rVue by LEVOIP’s clients. Upon receipt of such report, LEVOIP shall have sixty (60) calendar days in which to review the Utilization Report and submit a written dispute on activity contained therein, or asserting that activity has been omitted.

Utilization Report on or before September 1, 2002, the Company and the Administrative Agent shall agree upon a mutually acceptable monthly report reflecting the Company's utilization rates by equipment type for the immediately preceding calendar month (each a "Utilization Report").
