Utilization Report. (To be Completed by contractors and subs for all Projects at or exceeding $200,000)

In generating such reports, the Recipient may use or rely on State Water Board DBE Form UR-334 or USEPA Form 5700-52A. The Recipient must submit such reports to the Division annually by September 1 until such time as the “Notice of Completion” is issued. The Recipient must comply with 40 CFR § 33.301 and require its contractors and subcontractors on the Project to comply.

Utilization Report. Upon request, the supplier shall provide a usage report to the Purchasing Division. The report shall provide complete information regarding the quantities and sizes of desktop computers and monitors provided for the City under this contract.
