Utilization Report. (To be Completed by contractors and subs for all Projects at or exceeding $200,000)

Utilization Report. The Recipient must report Disadvantaged Business Enterprise (DBE) utilization to the Division. In generating such reports, the Recipient may use or rely on State Water Board DBE Form UR-334 or USEPA Form 5700-52A. The Recipient must submit such reports to the Division annually by September 1 until such time as the “Notice of Completion” is issued. The Recipient must comply with 40 CFR § 33.301 and require its contractors and subcontractors on the Project to comply.

Utilization Report. Upon request, the supplier shall provide a usage report to the Purchasing Division. The report shall provide complete information regarding the quantities and sizes of desktop computers and monitors provided for the City under this contract.
