Monroe County contracts for an aftercare program for individuals closed off of assistance for excess income. Monroe County has an MOU with City of Rochester to provide on site Financial Counseling through their Financial Empowerment Center model. This MOU will allow individuals we serve to access services related to budgeting, savings, and credit repair free of charge. MCDSS is continually monitoring our caseload of disabled individuals for solid medical documentation that could give a strong foundation for an application and award for SSI/SSD.

Described below are the district’s strategies for reducing the need for TA: To reduce the need for TA, OCDSS staff completes Employment Assessments with participants within 10 days of their eligibility interview. During the assessment barriers are addressed and job readiness and job search activities are started. OCDSS also focuses on preparing clients for job retention. Work readiness is an integral part of job search prep to ensure clients retain their jobs through good attendance, reliability, skill building and fulfillment of basic needs. Incentive cards and supportive services are offered to qualified participants to assist with obtaining and maintaining employment. Information regarding transitional benefits as well as other beneficial supports such as WIC, Head Start, Childcare Assistance Program, HUD, Child Support, National Grid budget plans, HEAP, free cell phone programs and free tax preparation is provided. Should a participant¿s Temporary Assistance case close due to earned income, participants are immediately determined eligible for our FFFS funded TANF 200% programs for the first 12 months following their closing. During the initial 12 months, monthly questionnaires are mailed to the participants to allow them the opportunity to identify any concerns they may be experiencing that the agency could assist with before they reach crisis status or impact their employment. Should a questionnaire be returned with concerns, a staff person reaches out to discuss possible resources to assist them. Each participant who returns their questionnaire and meets eligibility criteria is provided an incentive card to promote continued connection with the agency during this transition period. The goal of this ongoing case management service is to provided support for ongoing employment and reduce the need for Temporary Assistance.

Described below are the district’s strategies for reducing the need for TA: The district uses diversion strategies as follows; The Employment Examiner will meet with any/all individuals requesting "emergency" assistance or a diversion payment to enable them to maintain or obtain pending employment. Individuals are required to complete an application, provide necessary documentation to determine the household's eligibility and provide verification of the employment related issue. The individual must also provide verification of employment, verify available resources, and attest to the necessity of such expense. While the ultimate goal is to divert individuals from needing ongoing assistance, the worker will review and document whether there is a reasonable chance that the one-time short term payment will enable the applicant to maintain self-sufficiency and avoid the need for on-going assistance.

The Employment worker conducts the employment services orientation and an intake screening and then assigns the client to job search as a diversion. Child Care Assistance is authorized if necessary. Clients are referred to Rockland Works Career Center and Rockland BOCES to become job ready and for assistance in finding employment. A one- time Emergency Assistance benefit may be offered if appropriate in lieu of ongoing TA benefits.

Described below are the district’s strategies for reducing the need for TA: The TA/Employment workers assess the need for items listed below to divert an individual from ongoing assistance: Car purchase and repairs through the contracted Employment Related Transportationn Service provider or DCDCFS - for individuals who are verified to be working and do not live and work within two miles of public transportation routes or timetable or need a car to perform the job function (i.e. CNA). Individual applying for purchase of a car must work his/her scheduled hours for two full weeks prior to us purchasing the car. We will pay for alternate transportation for this time period if necessary. Car value must be at or below NADA average retail book value and the vehicle being purchased must be inspected by an independent certified mechanic. Rent and security to allow an individual to locate near a verified job. Bus passes, train fare, taxi fare for transportation to a job until first paycheck is received. Childcare to allow an individual to accept a verified job immediately. Tools or clothing to allow an individual to begin a verified job
