Self-Certification. For the Consultant and each subconsultant or subcontractor, the following must be completed. Indicate all Business category(ies) that apply by initialing next to the applicable category(ies): Small Business Enterprise (SBE) - an independently owned and operated concern certified, or Initial if applicable certifiable, as small business by the Federal Small Business Administration (SBA). (Size standards by Standard Industrial Classification codes required by the Federal Acquisition Regulations, Section 19.102, may be found at xxx.xxx.xxx/xxxx. The University may rely on written representation by the vendors regarding their status.) Annual average receipts, computed from the gross receipts for the last 3 fiscal years, do not exceed the amount listed in the MAXIMUM RECEIPTS TABLE below. Self-Certification. For each Proposed Product, Licensee must self-certify, at Licensee's expense, that such Proposed Product: (i) is compatible with all Compatible Products, as applicable, (ii) meets all requirements of the applicable Licensed Specifications and other Documentation, and (iii) complies with all other requirements set forth in this Use License. Licensee must document its self-certification of each Proposed Product with appropriate reports and supporting materials and will promptly make such documentation available to Apple upon request.
Self-Certification. The participant file must document participants’ eligibility. Because of the circumstances surrounding the disaster, documentation of eligibility may be difficult to obtain during the initial stages. The Department is prepared to accept an individual's signed certification that they meet the eligibility criteria. The Grantee should have a system in place to verify eligibility for individuals once better data are available. If the Grantee has such a system in place, and if a participant is later found to be ineligible, the costs incurred prior to the discovery of ineligibility will not be disallowed.

Self-Certification. No later than thirty (30) days prior to the Alternative Format Roll-out Date, each CRA will develop a method for Consumers to certify that they are Persons with a Visual Impairment. The form may include the following language: “I certify that I am a person who is blind or visually impaired within the meaning of the Americans with Disabilities Act.” The self-certification method shall be in the same format in which the Request for Disclosures has been made, or in another format that provides effective communication to the requesting Consumer and will not unreasonably delay processing the Request for Disclosure.
