Quarterly Performance Reports. The Contractor shall provide quarterly performance reports to the Department addressing the Contractor’s compliance with the performance measures, as applicable, and as stated in Section II., DD., of the Contract. If issues of non-compliance are identified in the quarterly performance reports, financial consequences will be assessed in accordance with Section II., EE., of the Contract. The quarterly performance reports shall be submitted by the tenth (10th) day of the month following the end of the corresponding quarter.

Quarterly Performance Reports. (i) Quarterly reports shall indicate the following information, pertaining to loans made pursuant to the 2012 Agreement and this Agreement, as a minimum: • percentage of both the number loans to and total investment into Target Markets; • internal leverage ratio for the FUND; • external leverage ratio for the FUND; • total matching funds raised, including source and terms of matching funds; • job creation/retention attributed to loans made pursuant to this Agreement; • list of loan recipients by organization’s name & contact person (to be kept confidential unless a waiver is received); • job creation/retention attributed to loan recipients that have signed a confidentiality waiver; and, • total company investment leverage attributed to loan recipients that have signed a confidentiality waiver.
Quarterly Performance Reports. These quarterly reports will detail the prior period’s progress on implementing the Agreement Addendum and shall include the county’s number of current active cases and contacts, how many LTBI cases initiated medication, how many LTBI cases completed medication, and when having provided DOT and DOPT, how many are provided face-to-face and how many are provided electronically. The first report is to report for June, July, and August 2022 activities and is due by September 22, 2022. Quarterly Performance Reports. The Recipient shall submit quarterly, cumulative, Performance Reports to the Human Services Department of the County within fifteen (15) business days following: December 31, March 31, June 30, and September 30. These reports should include but not limited to the number of unduplicated children served during the quarter, and the progress the agency has made toward meeting their goals and objectives as they stated in their RFP response.
Quarterly Performance Reports. The quarterly reports must include a data table for indicators and indicate progress achieved towards benchmarks, highlight tangible results and achievements identify any problems encountered in implementation and propose remedial actions as appropriate. The reports shall describe progress made during the reporting period and assess overall progress to that date versus agreed upon indicators including the agreement-level outputs achieved, using the agreement- level performance indicators established in the Initial/Annual Implementation Plan for the period being reported. The reports must also describe the accomplishments of the Contractor and the progress made and must include information on all activities, both ongoing and completed during the reported period. The reports must also discuss any potential constraints that might prevent the Contractor from meeting agreed upon targets and benchmarks and challenges that can impact overall implementation of the program. These reports must summarize the outcomes of the Contractor’s activities during the particular reporting period, document any program accomplishments or progress towards results during the reporting period, compare those results to the planned tasks in the Implementation Plan and Monitoring, Evaluation and Learning Plan, and discuss any potential constraints that might prevent the Contractor from meeting agreed upon targets and benchmarks. Quarterly performance reports must be submitted to the USAID/Ukraine COR in electronic format within ten (10) business days of the beginning of the months of January (Quarter 1), April (Quarter 2), and July (Quarter 3) of each fiscal year of implementation. An annual report will be provided in lieu of the fourth yearly quarterly report. One week following submission, or as agreed by the COR, the quarterly performance reports are to be reviewed in person with the COR and other representatives of USAID/Ukraine for the purpose of discussing ongoing activities and considering corrections or changes in the scope of work or implementation plan. Quarterly reports must include the following information, and any additional information as requested by the COR:
Quarterly Performance Reports. The award recipient is responsible for implementing mechanisms to monitor participants and report to the ECA Grants Office and the Program Office success and impact on a quarterly basis. An SF-PPR, “Performance Progress ReportCover Sheet, the SF-PPR-E, and SF-PPR-F must be submitted with all performance reports. Quarterly program and financial reports should include both quantitative and qualitative data.
