Psychological Testing. In the event the Employer has reasonable suspicion to believe that an employee is psychologically unfit for duty the Employer shall have the right to require the employee to undergo psychological evaluation subject to the employee’s rights under Illinois law.
Psychological Testing. Psychological testing is a covered Benefit when the Member is referred by an MHSA Provider, the procedure is prior au- thorized by the MHSA and when provided to diagnose a Mental Health Condition.

Psychological Testing. I understand I may be asked to complete a psychological evaluation and/or parenting assessment as part of this process. Test results, interview notes, reports, etc., shall be strictly confidential. Psychologist or other mental health professional shall be informed by the Employer that an employee may be deemed unfit for duty only if the employee's mental status poses a significant and direct present threat to his/her own health and safety, the safety of his/her co- workers, or the safety of the public. The psychologist, or other mental health professional shall notify the Employer of his/her opinion as to the employee's ability to perform the essential functions of his/her job, only after having been advised in writing by the Employer of those essential functions. The psychologist, or other mental health professional shall not disclose any case history, interview responses, test results or diagnoses supporting his/her opinion concerning the employee's ability to perform the essential functions of his/her job without the express written consent of the employee. Only licensed and certified mental health professionals trained and experienced in psychological test techniques and interpretation may conduct the testing. At no time may the mental health professional be advised by the Employer of any disciplinary, personal or other information or allegations against the employee. The purpose of this provision is to ensure the objectivity of the evaluator.
Psychological Testing. If clinically indicated, ▇▇. ▇▇▇▇▇▇▇ may refer to and utilize a qualified psychometrist to administer and interpret psychological tests as a supplement to his evaluation. Expert Opinion. Please be advised I will render an opinion based upon evidence, science, logic and clinical judgment. Thus, after evaluation of all the facts, it may be that the opinion offered is unfavorable to one or more claims or defenses that you and your client espouse.
Psychological Testing. A complete psychological evaluation is comprehensive and labor intensive. The face-to-face testing can take anywhere from 3 to 15 hours, depending on the diagnostic issue at hand. The scoring, interpreting, and writing of the report are typically completed around four weeks after the last testing session. Once completed, we will meet for the feedback session during which time I will review the findings, provide you with a copy of the report, and answer any questions that you may have. I evaluate various cognitive processes such as learning, memory, attention and concentration, and executive functioning. Additionally, I conduct assessments of the individual’s emotional functioning, which includes (but is not limited to) how they cope with difficult situations, their view of the world, their sense of self, and the degree to which they experience symptoms of depression and/or anxiety. As with all psychological services, testing is only as helpful as the individual allows it to be. Therefore, the degree to which insights can be made depends on the individual’s approach to the testing process. MEETINGS & CANCELLATIONS Over the course of our initial sessions, we can both decide if I am the best person to provide the services that you (or your child) need in order to meet your treatment goals. If psychotherapy is to be pursued, I will usually schedule one 45-minute session per week at a time we agree upon, although some sessions may be longer or more frequent. Regular attendance is a critical factor of a successful therapy. You are financially responsible for your appointments or for those of your child. Because unforeseen circumstances arise, however, the following policies have been adopted: • In general, I have a 48-hour cancellation policy. This means that I will hold you responsible for paying the full appointment fee if you cancel within 48 hours our scheduled time. However, some exceptions are noted below:

I charge $125.00 for each hour of time spent administering, scoring, and writing reports of test results. A $500 deposit is due on the date of testing, with the remainder due during the feedback session. I also require that you attend an initial interview appointment during which we will discuss testing options, schedules, and any other questions you might have. You should also check with your insurance company specifically about whether psychological testing for a particular issue is covered under your policy. Generally, insurance companies will not reimburse for testing to determine whether a learning disability is present. Other Professional Services (including phone conversations): I charge $125 per hour for other professional services you might need, though I will break down the hourly cost if I work for periods of less than one hour. Other services include report writing, telephone conversations lasting longer than 15 minutes, consulting with other professionals with your permission, school observations/consultations with your permission, preparation of records or treatment summaries, and the time spent performing any other service you may request of me.
