Provider Numbers. P.C. shall maintain provider numbers for Blue Cross, Blue Shield, Medicare and Medicaid (collectively "Provider Numbers"). To the extent practicable, group assignment accounts will be established to facilitate xxxxxxxx.
Provider Numbers. Each Shareholder holds a valid Medicare provider number and valid uniform physician identification numbers. Evidence of such numbers has been delivered to PSC.

BRFHH shall accept: (i) the Shreveport Hospital’s Medicare Provider Agreements and corresponding provider numbers 19-0098 and 19-S098; (ii) the Shreveport Hospital’s Medicaid Provider Agreements and corresponding provider numbers 1737712 and 1705675; (iii) X.X. Xxxxxx’x Medicare Provider Agreements and corresponding provider numbers 19-0011 and 19-S011; and (iv) X.X. Xxxxxx’x Medicaid Provider Agreements and corresponding provider numbers 1720372 and 1705128 (the foregoing collectively the “Provider Agreements”);
Provider Numbers. All Medicare and Tenncare/Medicaid provider numbers and provider agreements in connection with the operation of the MHP-TN REGION as described on Schedule 2.1.6.
Provider Numbers. The Practice and Seller holds a valid Medicare provider number, and Seller has a valid uniform physician identification number. Evidence of such numbers has been delivered to Parent.
Provider Numbers. Attached hereto as Schedule 2.17 is a list of all Medicaid and Medicare provider numbers (the “Provider Numbers”) in the name of the Hospital. The Provider Numbers are in full force and effect as of the date of this Agreement. Sellers shall reasonably cooperate with Purchaser with respect to the issuance of a new provider number.
Provider Numbers. The List of Provider Numbers attached hereto as Exhibit 6-U accurately and completely lists all of the provider numbers used by each of the Corporations.
