Preventative Services. All necessary procedures to prevent the occurrence of oral disease, including: Cleaning and scaling Topical application of fluoride Space maintainers iii) Surgical Services All necessary procedures for extractions and other surgical procedures normally performed by a dentist.
Preventative Services. Prophylactics (cleanings), fluoride treatments and oral hygiene instruction will be covered once every six (6) months. With respect to an employee who is absent from active employment as a result of sickness or injury or leave of absence, the Company will continue the full contribution for the employee’s dental benefit plan for six (6) calendar moths following the month in which disability or leave of absence begins. With respect to an employee who is absent from active employment due to a layoff or a reduction of forces, the Company will continue the fill contribution for such employee’s dental benefit plan through the end of the month in which the employee’s dental benefit plan through the end of the month in which the employee’s layoff commences.

Endodontic Services Endodontists specialize in root canal treatment and other procedures involving the tooth pulp. Root canal treatment involves removing the pulp and cleaning, sterilizing, filling and sealing the root canals. Your coverage for endodontic service include:  Treatment of pulp chamber.  Root canal therapy for permanent teeth.  Apexification and periapical services.

Preventative Services. Subscriber agrees to pay RFI $ annually in advance for the term of this agreement for preventative services. If this option is selected RFI will make inspection(s) per year. Unless otherwise noted in the EXHIBIT A: Schedule of Equipment and Services inspection service includes testing of all accessible components to ensure proper working order. If the system is UL Certified, the inspection will comply with UL requirements. RFI will notify Subscriber 3 days in advance of inspection date. It is Subscriber's responsibility to reschedule or permit access. Testing at inspection tests only that components are in proper working order at time of inspection unless otherwise reported to Subscriber at time of inspection. Inspection does not include repair. If Access Control doors are inspected, the inspection consists of the Access Control devices only. The inspection does not include inspection or testing of door leafs, frames, hinges, mechanical hardware, closers, or components of automated doors for which RFI has no responsibility or liability. ☐ (d) ON-SITE ADMINISTRATION SERVICES: Subscriber agrees to pay RFI the sum of $ annually in advance for the term of this agreement.
