Paramedical Coverage. The current paramedical coverage will have the maximum payment changed to five hundred dollars ($500) per year with each visit compensated at eighty (80%).

Paramedical Coverage. Effective August 3 2012: The Company will arrange to provide a combined benefit of up to $300 per year for existing paramedical services with the requirement that services must be prescribed by a Physician. (Chiropractic, Massage Therapy, Osteopathatic, Psychologist, Speech Therapist). Effective August 3, 2013, the combined amount will be increased to $350. The Company will arrange to provide a benefit of up to $150 per year for Massage Therapy coverage (for services prescribed by a Physician and preformed by a registered massage therapist). The Company will arrange to provide hearing aid coverage up to $500/5 years. Effective the pay period closest to the middle of November, 2008. The Company will arrange to provide a benefit for breast prostheses and wigs at the insurer’s reasonable and customary level. Effective the pay period closest to the middle of November, 2008.
