Monitoring and verification. For the purposes of implementing this Agreement, a Monitoring and Verification Mechanism (MVM) responsible for verifying compliance therewith and the administration of various factors that may jeopardise the BDCCH and LA, and in particular verifying compliance with the rules governing the BDCCH and LA will be set up. The functions, procedures and scope thereof are established in the MVM mandate. This is a flexible, efficient and rapid mechanism that will enhance the transparency and credibility of and confidence in the BDCCH implementation process. The international component of the MVM (IC-MVM) will verify the LA under the terms of and with the due guarantees established in the protocols to the Agreement. It has a structure composed of three units, one at national level; eight (08) regional verification structures; and local monitoring structures established in specific areas. The MVM will be a tripartite technical mechanism composed of representatives from the National Government (Colombian State Armed Forces, which include both the Military Forces and the National Police), the FARC-EP, and an international component comprising a political mission with unarmed UN observers made up principally of observers from the member states of the Community of Latin American and Caribbean States (CELAC). In all units, the international component leads the MVM and is responsible for settling disputes, submitting recommendations and generating reports, in accordance with the guidelines given to it in order to guarantee and support the impartiality and transparency of the BDCCH and LA. The number of monitoring teams will depend on the following criteria: areas, number of people and weaponry to be monitored, and topography and risk factors in each zone specified for said purpose. The MVM will establish links with social and political organisations and with communities, and also with state institutions at local, regional and national level and these may contribute to its work by providing information, by assisting in the dissemination of its reports to the public at large and by submitting proposals and suggestions.

Monitoring and verification. The SHA shall, either directly or through the Insurer / ISA / TPA/ISA or any of its authorised representatives, shall have the right to conduct monitoring visits and random audits of any or all cases of hospitalisation and any or all claims submitted by the EHCP. Monitoring of EHCPs shall include but not be limited to: Overall performance and conduct of the EHCP Beneficiary registration process Pre-authorisation and claims submission process EHCP facility and infrastructure. The scope of medical audit of services provided by the EHCP shall focus on ensuring comprehensiveness of medical records and shall include but not be limited to: Completeness of the medical records file Evidence of patient history and current illness Operation report (if surgery is done) Patient progress notes from admission to discharge Pathology and radiology reports. If at any point in time the SHA issues Standard Treatment Guidelines for all or some of the medical/ surgical procedures, assessing compliance to Standard Treatment Guidelines shall be within the scope of the medical audit. The SHA/TPA/ISA shall conduct the medical audit through on-site visits to the EHCP facility for inspection of records, discussions with the nursing and medical staff. The SHA/TPA/ISA shall conduct hospital audit of the EHCP that will focus on compliance to minimum empanelment criteria including but not limited to facilities, infrastructure, human resources, medical record keeping system and EHCP’s obligations like operational help desk, appropriate signage of the Scheme prominently displayed. The EHCP shall be obliged to provide unconditional support to the Insurer / ISA / TPA or any of its authorised representatives in all their monitoring activities which shall include but not be limited to providing access to the hospital facility, patients and record for planned and unplanned supervision visits, providing copies of all medical records of AB PM-XXX beneficiaries as required for purposes of audit or otherwise and any other cooperation and support that may be required under the provisions of this Agreement.
Monitoring and verification. 7.1. The Grantee shall immediately on request provide the Minister with any information, explanations and documents as the Minister may reasonably require in order to establish that the Grant monies have been used properly in accordance with this Agreement.
