Maximum Benefits. The annual contribution for a Participant’s HSA Benefits is equal to the annual benefit amount elected by the Participant. In no event shall the amount elected exceed the lesser of: • The statutory maximum amount for HSA contributions applicable to the Participant’s coverage option (single or family) for the calendar year in which the contribution is made, or • The annual deductible under the HDHP option elected by the Participant. An additional catch-up contribution may be made for Participants who are age 55 or older. • The maximum contribution shall be reduced by any matching Employer contribution made on the Participant’s behalf • The maximum contribution shall be prorated for the number of months in which the Participant is an HSA eligible individual.

Maximum Benefits. The maximum benefits We will pay are shown on the Schedule of Benefits of the Policy. We will not pay for Home and Community-Based Care on any day that You are confined in a Nursing Facility. Home and Community-Based Care benefits may be paid on any day You are confined in a Residential Care Facility provided the total benefits payable for that day will not exceed the Residential Care Facility Maximum Daily Benefit.

Maximum Benefits. Subject to the reinstatement provision, the maximum amount payable for any one (1) person while insured is Reinstatement Provision As of the second January 2nd following the effective date of this insurance, and on each January 2nd thereafter, of the original Major Medical maximum is rein- stated automatically. If, after this reinstatement, the unused portion of the maximum is less than an employee or qualified dependent may have the full maximum reinstated by submitting satisfactory evidence of insurabil- ity at the employee’s expense. Maximum Benefits. CSS, in its sole discretion, may pay a maximum of one thousand dollars ($1000.00) per benefit year per LPN in tuition assistance for LPNs who meet the tuition assistance eligibility criteria. This benefit may instead be used for registration, board and travel expenses arising out of attendance at a conference pursuant to Section 2 of this Article. This benefit will, however, be limited in the aggregate for LPNs covered by this Agreement to a total of fifteen thousand dollars ($15,000) per benefit year.
