Maximum Benefits. The annual contribution for a Participant’s HSA Benefits is equal to the annual benefit amount elected by the Participant. In no event shall the amount elected exceed the lesser of: • The statutory maximum amount for HSA contributions applicable to the Participant’s coverage option (single or family) for the calendar year in which the contribution is made, or • The annual deductible under the HDHP option elected by the Participant. An additional catch-up contribution may be made for Participants who are age 55 or older. • The maximum contribution shall be reduced by any matching Employer contribution made on the Participant’s behalf • The maximum contribution shall be prorated for the number of months in which the Participant is an HSA eligible individual.
Maximum Benefits. The maximum benefits We will pay are shown on the Schedule of Benefits of the Policy. We will not pay for Home and Community-Based Care on any day that You are confined in a Nursing Facility. Home and Community-Based Care benefits may be paid on any day You are confined in a Residential Care Facility provided the total benefits payable for that day will not exceed the Residential Care Facility Maximum Daily Benefit.
Maximum Benefits. The benefit year maximum is $1,000.00 for each benefit year for basic and major services. The lifetime maximum for orthodontic services is $1,250.00.
Maximum Benefits. During any one calendar year the maximum benefits payable for covered dental expenses, except for orthodontic treatment, is $1,000.00 for each person. Covered dental expenses in connection with orthodontic treatment will be a lifetime maximum of $1,500.00 for each dependent child under nineteen (19) years of age.
Maximum Benefits. Total additional increase for the final year of service of combined Basic and Other Benefits may not exceed 30% of the base contract salary for the final year of service. The base contract salary for an Administrator retiring during a contract year will be determined by prorating her/his salary over a full calendar year prior to the retirement date.

Maximum Benefits. Benefits paid to any Covered Person for dental expenses for Preventive, Basic, and Major Services in any one calendar year shall not exceed the Maximum Benefit therefor as specified in the "SCHEDULE OF BENEFITS". Orthodontic Services are also subject to a lifetime Maximum Benefit as specified in the "SCHEDULE OF BENEFITS".
Maximum Benefits. Subject to the reinstatement provision, the maximum amount payable for any one (1) person while insured is Reinstatement Provision As of the second January 2nd following the effective date of this insurance, and on each January 2nd thereafter, of the original Major Medical maximum is rein- stated automatically. If, after this reinstatement, the unused portion of the maximum is less than an employee or qualified dependent may have the full maximum reinstated by submitting satisfactory evidence of insurabil- ity at the employee’s expense. Carry-Over Provision Any eligible expenses incurred during the last three (3) months of a calendar year which were used to satisfy the Deductible Amount, either in full or in part, may be carried over into the following year to again satisfy the Deductible Amount. This provision eliminates the application of two (2) over the end and the beginning of a calendar year. Family Should more than one (1) member of one (1) family be- come eligible for Major Medical Insurance, the Deductible Amount in any calendar year shall be per family. Common Accident Provision In the event that two (2) or more insured members of a fam- ily are injured in the same accident, the Deductible Amount be applied only once for all their injuries arising out of
Maximum Benefits. CSS, in its sole discretion, may pay a maximum of one thousand dollars ($1000.00) per benefit year per LPN in tuition assistance for LPNs who meet the tuition assistance eligibility criteria. This benefit may instead be used for registration, board and travel expenses arising out of attendance at a conference pursuant to Section 2 of this Article. This benefit will, however, be limited in the aggregate for LPNs covered by this Agreement to a total of fifteen thousand dollars ($15,000) per benefit year.
Maximum Benefits. The annual maximum benefits paid out under the Plan for basic and/or major dental care per eligible person will be: - $ 2 300 For those whose coverage becomes effective after July 1, the combined maximum benefits for the remaining months of that year will be $1,150. Expenses Covered To be considered a covered expense, the charge for a particular service will be limited to the maximum fee set out in the dental fee guide published by the College of Dental Surgeons of the province in which dental services are provided. If dental treatment is rendered outside Canada, the benefits paid will be limited to the maximum fee set out in the dental fee guide published by the College of Dental Surgeons of the province in which the employee resides. The Plan covers dental treatment by dentists, physicians or other qualified personnel under the direct supervision of the dental or medical profession (e.g. dental assistants and dental hygienists). Basic Dental Care 100% reimbursable up to maximum limit - Oral examinations, cleaning of teeth, fluoride treatments and bite-wing X-rays: every nine (9) months. - Full-mouth series of X-rays: once every 24 months. - Extractions and alveolectomy (bone work) at time of tooth extraction. - Dental surgery. - General anaesthesia, diagnostic x-ray and laboratory procedures required for dental surgery. - Amalgam, silicate, acrylic and composite fillings. - Necessary treatment for relief of dental pain. - Cost of medication and injections administered in the dentist’s office. - Spacers for missing primary teeth and habit-breaking appliances. - Consultations prescribed by the attending dentist. - Surgical removal of tumours, cysts, neoplasms. - Incision and drainage of abscesses. 80% reimbursable up to maximum limit - Endodontics (root canal therapy). - Periodontal treatment (gum and tissue treatment).
