Letter of Agency. By entering into this Contract, you authorize XOOM to act on your behalf under Consumers Energy’s tariffs in accordance with the rules and regulations of the Michigan Public Service Commission and agree to the following statement: I acknowledge that I am the account holder or legally authorized person to execute a contract on behalf of the account holder. I understand that by signing this Contract, I am switching the gas Supplier for this account to XOOM. I understand that gas purchased for this account by XOOM will be delivered through Consumer Energy’s delivery system. The account holder, or the person who signed this contract on behalf of the account holder, has 14 days after today to cancel this contract for any reason through written or verbal notification to XOOM.I may waive this right of cancellation by affirmatively agreeing to this waiver on the contract. If you terminated your contract today, and if the unconditional cancellation period did not apply, based on current gas prices and your historical usage, a good-faith estimate of your termination fee would be $650. This termination fee is subject to change as your usage and the market price of gas fluctuate. You agree to authorize Consumers Energy to release all information relating to your historical and current gas usage, billing and payment history to XOOM or its authorized representatives. You acknowledge that XOOM has full authority to make all rates and tariff selections necessary to meet its obligations under this Contract.

Letter of Agency. If applicable, upon the execution of an applicable SOW, Customer will sign a Letter of Agencygiving CenturyLink the limited authority to directly notify the appropriate vendor for the purpose identified in the Letter of Agency. Customer may terminate this authorization at any time upon notice to CenturyLink. If applicable, Customer shall also provide its vendors with a letter (with a copy to CenturyLink) acknowledging CenturyLink’s role as Customer’s agent solely as it relates to the purpose as specifically identified in such letter of agency. As soon as commercially practicable, Customer shall provide CenturyLink with a copy of any contractual commitments between Customer and its vendor that CenturyLink must be aware of or comply with in order to dispatch such vendor accordingly.
Letter of Agency. In the event MCI WorldCom handles Customer's PIC order processing, Customer agrees to execute an LOA in a form substantially similar to Exhibit "B" attached hereto or such other documents as may be required by the LECs to effect the PIC order processing and give MCI WorldCom the right to obtain information from the appropriate LEC regarding Customer (i.e., data gather) and to provide the LECs with an appropriate LOA designating MCI WorldCom as its agent for purposes of handling Customer's PIC order processing. MCI WorldCom agrees to provide status reports to Customer concerning such PIC order processing. Customer shall be solely responsible for resolving disputes with the LECs attributable to Customer's CIC. Further, Customer shall reimburse MCI WorldCom for all third party costs incurred by MCI WorldCom in handling such PIC order processing.

(“LCSI”) to act on behalf of (“Customer”) for the purposes of updating and managing our Automatic Location Identification (“ALI”) data in the master ALI database of record for the region(s) in which we conduct business. LCSI is authorized to communicate with our Telecommunications Service Provider(s), the ALI DBMS administrator of record, and any others as necessary to manage our ALI data in conformance with LCSI’s ALIProWeb™ service as described on LCSI’s ALIPro™ product web site, xxx.XXXXxx.xxx. Letter of Agency. This confirms Sedona Oak Creek Unified School District No. 9’s participation with and representation by Yavapai County through the Yavapai County School Superintendent (hereinafter referred to as “YCSS”) for procurement of affordable telecommunications and/or Internet access, and related equipment. I hereby authorize YCSS to submit FCC Form 470, FCC Form 471, and other E- rate forms to the Schools and Libraries Division of the Universal Service Administrative Company on School’s behalf. I understand that, in submitting these forms on School’s behalf, YCSS is making the certifications for School. By signing this Letter of Agency, I make the following certifications:
Letter of Agency. Reseller shall promptly obtain a written Letter of ---------------- Agency ("LOA") (or any other means approved by the Federal Communications Commission, hereinafter "FCC") acceptable to T-NET for each ANI indicating the consent of each End User to be served by Reseller and transferred to Reseller via a change of such End User's designated primary interexchange carrier (hereinafter "PIC") prior to Reseller submitting the Service Request to T-NET for such End User's order. Each LOA shall include, among other things, (i) that an End User Subscriber has consented by an authorized and dated signature of End User subscriber to the transfer of its long distance service to Reseller, (ii) that an End User Subscriber shall be billed a charge for the transfer of its long distance service to Reseller and that such charge shall appear on its local service provider xxxx, (iii) an End User Subscriber's billing address, (iv) the main ANI and other ANIs of an End User Subscriber that are being transferred to Reseller, and (v) the written authorization of an End User Subscriber to transfer its secondary long distance service to Reseller if that End User Subscriber is located in an area which allows for a secondary long distance provider. Upon T-NET's request, Reseller shall, within two (2) business days of receipt of such request, provide copies of the documentation that Reseller uses to satisfy the above. Reseller shall comply with all laws and regulations including all FCC rules such as 00 X. X.X, XXXX 00, Xxxxxxx X, (X)00.0000 through (S)641150 regarding LOAs and PIC transfer requirements and 47 C.F.R, Part 64, Subpart K, (S)64.11 regarding the verification of orders for long distance service generated by telemarketing.
