Formal Application. In order to participate in the plan, an eligible employee must make written application to the Board on or before March 31 of the current school year, stating the date when the eligible employee wishes the monthly deferrals (deductions) to commence.

Once the date is cleared with the Church Administrator, the request for reservations giving specific dates and exact time event must be made in writing and be submitted to the Church Administrator on the Request to the Session (Form A) for approval by the Session.
