Final Evaluation Report. The state must submit to CMS a draft of the evaluation final report within 60 days prior to the expiration of the demonstration. Final Evaluation Report. The final evaluation report is due to, and must be submitted to Health Services and Human Resources fully completed and signed (or acknowledged) by May 15. At least five (5) work days prior to the submission deadline the evaluator must provide a draft evaluation report to the nurse and be available for conference or discussion to clarify any questions or concerns. The content of the final evaluation report must have been presented to and reviewed with the nurse prior to submission.

The final evaluation report is due within 30 days of the end of the Term. Client Demographic Form: Contractor shall complete and submit a Client Demographics form (see Exhibit E) at the end of the Term. The Client Demographic report is due at the same time as the Evaluation and End-of-Year Financial Report. If your grant specifically targets a certain demographic of people within the services provided, you must collect demographic data in that category of the report form, to illustrate that the project is reaching the specific client population you have been contracted to serve.

Such signature does not signify agreement in whole or xxx with the report but merely that the teacher had an opportunity to discuss the report and has received a copy.
