Subscriber acknowledges that the Company has offered to make available any additional public information that the Subscriber may reasonably request, including technical information, and other material information about the Company and Subscriber has been offered Company's full cooperation in making such information available to Subscriber and acknowledges that the Company has recommended that the Subscriber request and review such information prior to making an investment decision. No oral or written representations have been made, or oral or written information furnished to the undersigned or its advisors, if any, in connection with the offering of the Securities which were or are in any way inconsistent with the SEC Filings.

Business Associate agrees to document such disclosures of Protected Health Information and information related to such disclosures as would be required for Covered Entity to respond to a request by an Individual for an accounting of disclosures of Protected Health Information in accordance with 45 CFR Section 164.528. Within 10 days of receipt of a request from the Covered Entity, Business Associate agrees to provide to Covered Entity or an Individual information collected in accordance with this subsection to permit Covered Entity to respond to a request by an Individual for an accounting of disclosures of Protected Health Information in accordance with 45 CFR Section 164.528.

Disclosure Documentation. The Company will provide to Special Warrantholders copies of all documentation required to be provided by applicable law to registered holders of Common Shares, as if such Special Warrantholders were registered shareholders of the Company.
Disclosure Documentation. Qlik shall document disclosures of PHI and information related to such disclosures as as and to the extent required of a Business Associate under HIPAA to respond to a request by an Individual for an accounting of disclosures of PHI in accordance with the Individual’s right to receive such accounting under 45 C.F.R. § 164.528. On written request Qlik shall provide to Customer such reasonable information to permit Customer to respond to a request by an Individual for an accounting of disclosures of PHI in accordance with the Individual’s right to receive such accounting under 45 C.F.R. § 164.528.
Disclosure Documentation. Document disclosures of PHI made by Business Associate, which are required to be accounted for under 45 CFR §164.528(a)(l), and make this information available as necessary to satisfy the Covered Entity's obligation to provide an accounting of disclosures to an Individual within five (5) business days by the Covered Entity of a request by an Individual of a request for an accounting of disclosures of PHI. If an Individual directly requests an accounting of disclosures of PHI from Business Associate, Business Associate shall notify Covered Entity's University System HIPAA Privacy and Security Officer(s) and [UH COVERED COMPONENT’s] Unit HIPAA Coordinator of the request within five (5) business days, and Covered Entity shall either direct Business Associate to provide the information directly to the Individual, or it shall direct that the information required for the accounting be forwarded to Covered Entity for compilation and distribution to the Individual, 45 CFR §§ 164.504(e)(2)(ii)(G) and (H), 164.528.
