COLLECTION PROCEEDINGS. WeBill retains the right to institute collection proceedings in a court of law of competent jurisdiction for matters involving outstanding payment.
COLLECTION PROCEEDINGS. Should my account become delinquent, I will be responsible for additional expenses to collect on my account, including reasonable legal fees, collection costs, and other expenses reasonably incurred.
COLLECTION PROCEEDINGS. The parties acknowledge and agree that this agreement contemplates work to be done by the Firm that requires substantial investment of Firm funds and time, that the decisions to undertake such work are highly subjective in nature and are made based upon the Firm’s investigation of the debtor’s financial position. For this reason the Firm retains the right to undertake or not undertake any and all collections proceedings as it deems financially viable based on the information developed during its investigation of the debtor. The Firm agrees, however, upon the separate written request of the Client, to provide reasonable verbal or email notification the Client prior to commencement of any particular post- judgment proceeding, e.g., real or personal property levy, garnishment, attachment, etc..

By signing below you acknowledge and understand that in the event your account is turned over to a collection agency for non-payment or other delinquency, you will be responsible for payment of any and all reasonable collection costs and/or attorney fees, in addition to the balance owed. All accounts turned over to a collection agency forfeits any past special fees and/or discounts. Such special fees and/or discounts will be reversed and you will be responsible for payment of regular fee for procedures at the time of service.

FAILED/MISSED APPOINTMENTS Failed appointments are a significant contributor to rising health care costs. We require at least 48 hours notice to cancel an appointment. If 48-hour notice is not given, your appointment is considered a MISSED appointment. Missed appointments will be assessed a fee of $50 per half hour of the missed appointment. This fee is due by the patient. After 2 missed appointments, you will be on a WALK-IN BASIS ONLY. Due to the hardship caused by repeated missed appointments, dental care may be TERMINATED and your insurance carrier will be notified. DENTAL RECORDS Your records can be released upon receipt of a dental records release form from the provider you wish to have your records send to. If you would like a copy of any records, there is a charge of $25. ACKNOWLEDGEMENT OF RECEIPT OF PRIVACY PRACTICES I have received a copy of this office’s Notice of Privacy Practices. I further understand that I have the right to refuse to sign this acknowledgement. COLLECTION PROCEEDINGS. In the event that the Owner fails to pay all amounts owed to OBC under this Agreement upon demand, OBC may take the following actions against the Owner: 1) demand an interest rate of 24% per annum, or the highest rate permitted by applicable law, on all past due amounts owing; 2) the Owner agrees to pay any and all costs incurred by OBC in seeking to collect such outstanding amounts, including but not limited to, actual legal fees, administrative fees, cost of recovery, insufficient funds fees and collection fees; 3) the Owner permits OBC, and its collection agents, to access any and all personal information provided by the Owner to OBC in an effort to collect any amounts owing under paragraph 18; and 4) OBC may report any deficiencies owed by the Owner to an appropriate credit reporting agency.
COLLECTION PROCEEDINGS. If your account becomes delinquent, you agree to pay CASA for any expenses CASA incurs to collect on your account, including reasonable attorneys’ fees and collection costs.
COLLECTION PROCEEDINGS. In the event it is necessary for Attorney to institute collection proceedings or litigation for the fees and expenses set forth herein, Client agrees to pay any and all legal fees and costs incurred by Attorney applicable thereto, whether or not a formal action is deemed necessary to be filed by Attorney's legal counsel.
COLLECTION PROCEEDINGS. To file any claims or take any action or institute any proceedings which the Secured Party may deem necessary or desirable for the collection of any of the Collateral or otherwise to enforce the rights of the Secured Party with respect to any of the Collateral.
COLLECTION PROCEEDINGS. In the event which CUSTOMER does not remit payment within thirty (30) days after the Due Date, MTS reserves the right to contract with a third party for execution of collection proceedings on behalf of MTS.
