Claim Information. As per clause H7.4, in respect of each ARRC Claim you submit under this Agreement, you must provide the NHI number of each Service User who was registered with your Pharmacy as receiving ARRC Pharmacy Services. From an indicative date of 1 February 2013 (subject to amendment after applying the Transitional Change Mechanism provisions in clause L7), you must also provide the DoB of each Service User in respect of all claims covered in clause H7.
Claim Information. Each Party shall promptly:
(a) inform the other by written notice of any actual or threatened Claim to which Sections 15.1 or 15.2 apply;
(b) provide to the other Party copies of all papers and official documents received in respect of any such Claim; and
(c) cooperate as reasonably requested by the other Party in the defense of any such Claim.
Claim Information. Each Party shall promptly:

Claim Information. 6.2.1 Claimant is responsible for providing sufficient information to substantiate Customer's rights to remedies under this Exhibit C. Boeing may reject a claim for lack of sufficient information. At a minimum, such information must include:
(i) identity of claimant;
(ii) serial or block number of the aircraft on which the defective Boeing Product was delivered;
(iii) part number and nomenclature of the defective Boeing Product;
(iv) purchase order number and date of delivery of the defective spare part
(v) description and substantiation of the defect;
(vi) date the defect was discovered;
(vii) date the Correction was completed;
(viii) the total flight hours or cycles accrued;
(ix) an itemized account of direct labor hours expended in performing the Correction; and (x) an itemized account of any direct materials incorporated in the Correction.
6.2.2 Additional information may be required based on the nature of the defect and the remedies requested.
