Case Records. Contractor shall preserve all case documents, notes, files, physical evidence or any other items created or received in the course of the representation of a client in an orderly and organized manner such that it can readily be made available to successor counsel, if one is appointed or retained. Case Records. 10 5.1 CONTRACTOR shall maintain a current and complete electronic case 11 record on COUNTY’s computer information system for each Participant referred. 12 ADMINISTRATOR will provide sufficient training regarding use and maintenance 13 of electronic case records on the computer information system, track cases, 14 generate reports, etc., to CONTRACTOR’s staff. CONTRACTOR shall conduct 15 future training for its staff and also provide this training to any partner 16 agencies that will have on-line access.
Case Records. Counsel shall compile and maintain appropriate case records for each person whom Counsel is appointed to represent. Counsel shall maintain such case records in their entirety for a period of no less than seven (7) years from the date on which the case or matter is fully and finally concluded or for any other time period specified under applicable court rule or statute, whichever date/event occurs last.

Health Insurance Portability and Accountability Act (HIPAA) Releases as needed. HEP if provided by PROGRAM PARTICIPANT. Program application. Housing agreement. TILP and amendments. Intake Summary. Needs and Services Plan and updates. Interviews with PROGRAM PARTICIPANT. Special Incident Reports. Social history report. Monthly evaluations. Progress notes, school performance, completed vocational assessment and employment attainment and progress. PROGRAM PARTICIPANT’s financial information, including revenues and disbursements for clothing and material provided by COUNTY and signed for by PROGRAM PARTICIPANT; wages and other incomes; allowances and incentives received by and signed for by PROGRAM PARTICIPANT. Documentation of all services provided, including contacts with and on behalf of PROGRAM PARTICIPANT and general observations. Documentation of community organizations working with PROGRAM PARTICIPANT. Child care arrangements/documentation. Documentation/justification for Supportive Services. Documentation of hours of participation. Documentation regarding any cooperation issues and cause determinations. Family connections. Employment information and employment retention tracking. Documentation of changes in earnings. Standard release forms as needed for collateral contacts. Documentation of language needs and how they were addressed, as applicable. Copies of rights and responsibilities and other forms and documents required CDSS, CCLD, ADMINISTRATOR, or in program procedures. Medical verifications, as applicable. Medical/dental records of visits/treatment. Quarterly Performance report. Termination Summary.
Case Records. You agree that none of the case records (tests, interview notes, observations notes, and notes from others) will be given to either parent (party) until the evaluation is finished. Then, the lawyers of the parents (parties) may look through and ask for copies of these records. The tests, though, cannot be copied. The tests can only be given to another psychologist. Copying fees for all case records are 96 cents per page for the first 30 pages and 73 cents per page after that. You also agree to pay $25 for a secretary to copy the records. In addition, you will pay a $40 per hour case review fee. This covers the cost of the time Xx. Xxxx spends with you while you review the file. Xx. Xxxx will not discuss your case with you during your case review appointment. Will I get the report? At the end of the evaluation, your lawyer and Guardian ad litem will be given a written report with recommendations. Can I know what Xx. Xxxx is going to say? Xx. Xxxx’x findings and opinions are important to you. Please do not ask Xx. Xxxx to give you any information until Xx. Xxxx has completed the evaluation process. Xx. Xxxx must look at the tests and all the information necessary to complete a thorough evaluation. You will get to hear and respond to the results of the evaluation about you before the report is finished. Then, your lawyer and the Guardian ad litem will receive the report. Legal facts Xx. Xxxx’x statements, report, and other actions might harm your court case.
Case Records. Provider shall maintain client records in a way that evidences compliance with the terms of this contract and laws pertaining to this contract.
Case Records. 7.2.1 CONTRACTOR shall maintain an imaged (electronic) case record. The content of the case records shall be in a format approved by ADMINISTRATOR. The case record shall contain any documentation not included in COUNTY data system.
