Effective for in-service withdrawals applied for after December 31, 1992, the Participant shall be provided the notice prescribed by Code section 402(f).

The application must clearly state the intended remaining degree of work. At the same time as the application is handed to the employer the employee shall also notify the local union branch. No later than two months after the employer has received the application the employer must reply in writing to the salaried employee and the local union branch as to whether the application has been granted or not, unless the em- ployer and salaried employee have agreed on a postponement of reply. Not an- swering within the stated time is a violation of a rule of order and does not have the effect that the application is deemed to have been granted.
