AGREEMENT TYPE. This is a Basic Ordering Agreement (BOA), Fixed Unit Price (FP) Agreement titled as the “CHSi Special Account Program Agreement”. It is referred to as Agreement throughout the rest of this document. Any supplies and services to be furnished under this Agreement shall be ordered via CHSi, of which performance against constitutes acceptance of this order. Under this agreement, TeamCME members will be furnished a requisition number by CHSi for each group of services to be performed on a specific CHSi client employee. Each requisition shall serve as a separate order for services under this Basic Ordering Agreement.
