The Claims Administrator may, at any time, request from the claimant, in writing, additional information as the Claims Administrator may reasonably require in order to adequately evaluate the Settlement Claim.
Members seeking to submit a Settlement Claim under this Agreement (other than Police Officer Settlement Class Members’ $175.00 cash payment) must complete and submit a written Claim Form to the Claims Administrator, postmarked on or before the Claims Filing Deadline.

A Settlement Class Member who elects to submit a Settlement Claim for reimbursement of Out-of-Pocket Losses must also provide to the Settlement Administrator information required to evaluate said claim, including: (1) documentation reasonably supporting his or her claim, and (2) a brief description of the nature of the loss, if the nature of the loss is not apparent from the documentation alone.
